Mac Intyre is seeking a My Way Facilitator (Transitions Co-ordinator) to support young people and families as they move through the No Limits provision.
You will build relationships, coordinate transition planning, and work with partners to ensure positive outcomes.
The role requires empathy, ability to travel across Nuneaton and surrounding areas, and experience in education/health/social care for young people with learning disabilities or autism.
