Location: Telford Area

Pay Rate: £16.35 per hour PAYE

Hours: 37 hours per week, Monday to Friday, 9:00am - 5:00pm

Contract: Temporary until 31 August 2026

We are currently recruiting for an experienced Adult Practitioner to support a busy Adult Social Care team. This role offers a hybrid working arrangement, with office attendance required on Mondays and a combination of home working and client visits throughout the week.

Key Responsibilities

  • Complete Care Act assessments, reviews and reassessments.
  • Carry out Carers Assessments and Mental Capacity Assessments.
  • Develop strengths-based, person-centred support plans.
  • Promote independence, wellbeing and informed choice.
  • Support access to community resources, assistive technology and digital solutions.
  • Manage risk and contribute to safeguarding activities.
  • Maintain accurate case records and documentation.
  • Work collaboratively with families, carers, partner agencies and multi-disciplinary teams.
  • Support individuals to access personal budgets and direct payments.

Requirements

  • Experience working within Adult Social Care.
  • Strong knowledge of the Care Act, Mental Capacity Act and safeguarding principles.
  • Experience completing reviews, reassessments and support planning.
  • Excellent communication and assessment skills.
  • Enhanced DBS with Adult and Child Barred List clearance.
  • Full UK driving licence and access to a vehicle.
