We are currently recruiting for a compassionate and dedicated Personal Advisor to join a busy Leaving Care Service, supporting young people as they transition into independent adulthood. This rewarding role involves managing a diverse and complex caseload of care leavers, including former and current asylum-seeking young people, individuals with unresolved immigration status, and young people with disabilities. You will play a vital role in helping young people achieve positive outcomes by providing practical, emotional, and professional support tailored to their individual needs.

Working collaboratively with a range of agencies and professionals, you will build trusted relationships, develop outcome-focused plans, and empower young people to develop the skills and confidence needed for independent living. An Enhanced DBS for Children and Adults is required for this role. You will also need to be a driver.

Key Responsibilities:
  • Manage a diverse and complex caseload of young people leaving care.
  • Complete comprehensive needs assessments and develop person-centred support plans.
  • Produce SMART, outcome-focused Pathway Plans in line with statutory requirements.
