Pay Rate: £16.35 per hour PAYE
Hours: 37 hours per week, Monday to Friday, 9:00am - 5:00pm
Contract: Temporary until 31 August 2026
We are currently recruiting for an experienced Adult Practitioner to support a busy Adult Social Care team. This role offers a hybrid working arrangement, with office attendance required on Mondays and a combination of home working and client visits throughout the week.
Key Responsibilities
- Complete Care Act assessments, reviews and reassessments.
- Carry out Carers Assessments and Mental Capacity Assessments.
- Develop strengths-based, person-centred support plans.
- Promote independence, wellbeing and informed choice.
- Support access to community resources, assistive technology and digital solutions.
- Manage risk and contribute to safeguarding activities.
- Maintain accurate case records and documentation.
- Work collaboratively with families, carers, partner agencies and multi-disciplinary teams.
- Support individuals to access personal budgets and direct payments.
Requirements
- Experience working within Adult Social Care.
- Strong knowledge of the Care Act, Mental Capacity Act and safeguarding principles.
- Experience completing reviews, reassessments and support planning.
