We are currently recruiting for a compassionate and dedicated Personal Advisor to join a busy Leaving Care Service, supporting young people as they transition into independent adulthood. This rewarding role involves managing a diverse and complex caseload of care leavers, including former and current asylum-seeking young people, individuals with unresolved immigration status, and young people with disabilities. You will play a vital role in helping young people achieve positive outcomes by providing practical, emotional, and professional support tailored to their individual needs. Working collaboratively with a range of agencies and professionals, you will build trusted relationships, develop outcome-focused plans, and empower young people to develop the skills and confidence needed for independent living. An Enhanced DBS for Children and Adults is required for this role. You will also need to be a driver.

Key Responsibilities

  • Manage a diverse and complex caseload of young people leaving care.
  • Complete comprehensive needs assessments and develop person-centred support plans.
  • Produce SMART, outcome-focused Pathway Plans in line with statutory guidance and organisational procedures.
  • Support young people to achieve greater independence and successfully transition into adulthood.
  • Build positive, trusting relationships that encourage engagement and long-term support.
  • Work collaboratively with social workers, education providers, health professionals, housing services and partner agencies to deliver effective care planning.
  • Promote the participation of care leavers in decisions affecting their lives and the ongoing development of the Leaving Care Service.
  • Understand and respond to the individual needs of each young person, recognising diversity, culture, disability and personal circumstances.
  • Work independently with young people and, where appropriate, their families and support networks.
  • Produce clear, accurate and timely reports and maintain high-quality case records.
  • Use supervision, professional judgement and team discussions to support effective decision-making.
  • Maintain professional standards through continuous learning, development and training.

About You

  • Experience working with vulnerable young people, care leavers, or children and families.
  • A good understanding of the challenges faced by young people transitioning to independence.
  • Knowledge of relevant legislation, statutory guidance and safeguarding practices relating to leaving care.
  • Excellent assessment, planning and report-writing skills.
  • The ability to build positive, professional relationships based on trust and respect.
  • Strong organisational skills with the ability to manage a demanding and varied caseload.
  • Excellent communication and partnership-working skills.
