We are seeking a full-time Licensed Social Worker to work with our clients in Adams, Franklin, and York Counties.
General Responsibilities: This is a professional position responsible for providing a wide range of psychosocial services to terminally ill patients and their families, including completing comprehensive psychosocial assessments and regularly assessing the patient and family psychosocial needs. The social worker also completes bereavement risk assessments and provides counseling related to anticipatory grief issues. A psychosocial care plan is developed with the Interdisciplinary Team (IDT) and the patient/family, which is implemented and continually revised based on ongoing assessments. Services may include supportive counseling, problem solving, referrals to community resources, pre-bereavement counseling, and support.
Education/Training/Certifications:
- Master’s Degree in Social Work (MSW) from an accredited graduate school.
- Current license as a Social Worker by the Pennsylvania State Board of Social Work Examiners.
- Valid driver’s license, automobile in good condition, and automobile insurance compliant with PA laws.
Skills and Knowledge:
- Sufficient visual/hearing ability to comprehend written and verbal communication.
- Excellent verbal communication skills.
- Ability to effectively manage stress.
- Manual dexterity for writing, keyboarding, and telephone operation.
- Ability to work in various settings, including patients’ homes and facilities.
Experience:
- Understanding of hospice philosophy and principles of death and dying.
- Hospice or home care experience preferred.
- Ability to assess patient needs and develop individualized care plans.
- Ability to work in the field and make sound judgments.
- Effective communication, documentation, interpersonal, and computer skills.
