Lupset Health Centre is looking for a Medical Care Navigator to join our friendly and dedicated practice team. Job Responsibilities:

  • Care‑navigate patient appointment requests by telephone, online or in person to the appropriate clinical person.
  • Greeting patients and noting their attendance at the surgery for a booked appointment.
  • Deal with visit requests.
  • Registration of new patients and temporary residents.
  • Process patients' change of address details.
  • Process repeat prescription requests in accordance with practice guidelines.
  • Having full understanding of the appointment system.
  • Have a clear understanding of telephone system, daytime and out of hours.
  • Computer data entry – processing and recording information in accordance with practice procedures.
  • Answering the telephone promptly.
  • Taking messages for members of the primary health care team, ensuring that urgent messages are given highest priority and delivered promptly.
  • Taking payments for services over the counter, ensuring that a receipt is issued on each occasion.
  • Continuously updating knowledge of service through information provided at staff training, meetings, emails etc.
  • Chaperone for clinical staff when required.
  • Clinical recall for patients that require annual reviews for long‑term conditions.
  • Complete workflow tasks within agreed timescale.

Qualifications:

  • 4 GCSE grade C or above
  • Disclosure and Barring Service Check
