We are seeking dedicated, empathetic, and motivated Support Workers to join our team supporting young people aged 15-19 transitioning from care into independent living. This is a highly rewarding role focused on empowering young people to develop the confidence, resilience, and practical life skills needed to successfully move into adulthood and live independently.

Key Responsibilities

  • Provide practical and emotional support to young people leaving care
  • Assist with developing independent living skills including cooking, budgeting, cleaning, and tenancy management
  • Support with education, training, employment, and career planning
  • Build positive, trusting relationships to promote emotional wellbeing and confidence
  • Encourage engagement with community resources and social opportunities
  • Work alongside social workers, personal advisors, and external professionals
  • Maintain accurate records, support plans, and progress notes
  • Promote safeguarding, wellbeing, and positive outcomes for all young people
  • Support young people through challenging behaviours, crisis situations, and emotional difficulties where required

Requirements

  • Experience working with young people, care leavers, or vulnerable adolescents preferred
  • NVQ/QCF Level 3 in Health & Social Care / Children & Young People (desirable)
  • Understanding of challenges faced by care-experienced young people
  • Strong communication, mentoring, and relationship-building skills
  • Patient, resilient, and proactive approach
  • Full UK Driving Licence preferred but not essential
  • Enhanced DBS required (or willingness to obtain)
