Join Our Team We are looking for a compassionate, reliable, and motivated Outreach Support Worker to join our dedicated team. In this rewarding role, you will support adults with low-level mental health needs and or learning difficulties , helping them to live as independently as possible within their local community.

Key Responsibilities Provide high-quality, person-centred support tailored to individual needs.
Support individuals to develop and maintain independent living skills.
Assist with accessing the community, attending appointments, and participating in social activities.
Promote choice, dignity, independence, and inclusion at all times.
Support individuals with emotional wellbeing and low-level mental health needs.
Maintain accurate records and complete daily documentation.
Work collaboratively with colleagues, families, and other professionals to achieve positive outcomes.
Follow safeguarding procedures and organisational policies.
Essential Requirements A minimum of 12 months' experience in a similar support worker or outreach role.
Experience supporting adults with learning difficulties and/or mental health needs.
A full UK driving licence and access to a vehicle is essential , as this role involves travelling between service users in the community.
Excellent communication and interpersonal skills.
A caring, patient, and positive approach.
The right to work in the UK.
