Job Summary

Lower Broughton Health Centre is looking for an enthusiastic individual to add to our supportive multi-disciplinary team of GPs, Practice Nurses, Health Care Assistants, Practice Pharmacists and Admin Staff. This role is ideal for a nurse who is passionate about chronic disease management and health promotion, and who has a keen interest in professional development.
Main Duties of the Job
- Chronic Disease Management: Leading clinics for conditions such as Asthma, COPD, Diabetes, and Hypertension. Assessing, implementing and evaluating individual treatment plans for patients with known long-term conditions or at risk of long-term conditions.
- Cervical Cytology: Performing screening and providing advice on womenβs health.
- Family Planning and Menopause care: Supporting and advising patients requesting information relating to family planning needs and Menopause Health.
- General Treatment Room Duties: Including immunisations and travel vaccinations (childhood and adults), and NHS health checks.
- Health Promotion: Empowering patients to manage their own health through education and lifestyle advice.
- Supporting vulnerable patients with learning disabilities and dementia.
- Maintaining disease registers.
- Supporting nursing colleagues in maintaining infection control logs.

Person Specification
Essential: Registered General Nurse (NMC), Motivated, Organised and excellent communication skills & empathy. Willingness to learn. Consideration may be given to nurse applicants who are experienced Community or secondary care nurses, particularly but not exclusively from those with diabetes or respiratory disease experience, who wish to move to primary care with training supported.
Desirable Qualifications: Primary care experience, Independent Prescriber (or willingness to train), Experience using EMIS Web / Docman, Experience of Cervical Cytology & Chronic Disease Management, Proactive approach to QOF targets.
