Main Duties

As a Band 5 Speech and Language Therapist (SLT) on the Adult Community Neuro Rehabilitation Team you will support patients with a range of communication and swallowing difficulties linked to neurological conditions. You will work both domiciliary and outpatient settings, completing assessments, providing therapy and developing plans for rehabilitation assistants. Additional responsibilities may include leading groups, training staff and contributing to quality improvement projects.

Your key duties will include:

  • Managing an independent caseload using evidence‑based practice.
  • Collecting, analysing and interpreting case‑related information.
  • Performing formal and informal assessments for speech, language, communication disorders (SLCD) and dysphagia.
  • Formulating differential diagnoses and developing patient‑centred management plans.
  • Providing therapy, supervising rehabilitation assistants and recording progress.
  • Attending MDT meetings and maintaining clear clinical reports.
  • Handling triage, management and discharge of patients without supervision.
  • Communicating complex information in appropriate modalities to patients, carers and health professionals.
  • Monitoring patient progress and adjusting treatment plans as needed.
  • Identifying and communicating risk factors for impaired swallowing and communication.
  • Referring to specialist SLT intervention where required.
  • Training and advising patients, carers and non‑professionals on speech, language and swallowing support.

Person Specification – Experience

Essential

  • Autonomous practitioner with experience of managing a caseload.
  • Experience in cognitive neuropsychological assessment and treatment planning.
  • Experience in dysphagia management.
  • Experience working in a multi‑disciplinary team in a rehabilitation setting.
  • Evidence of CPD.

Desirable

  • Experience working with staff from other agencies.
  • Experience teaching or training.
  • Experience working with long‑term conditions.
  • Placement experience in a relevant setting.

Person Specification – Qualifications

Essential

  • Degree or equivalent in speech and language therapy.
  • HPC Registration.
  • RCSLT Registration.
  • RCSLT Level C dysphagia competencies (completed).
  • Evidence of ongoing education and professional development.
  • Computer literate.
  • Recognised qualification in written and spoken English.

Desirable

  • Membership of relevant Clinical Excellence Network.
  • RCSLT accredited post‑graduate dysphagia training or willingness to undertake.

Person Specification – Knowledge

Essential

  • Excellent knowledge of SLCD differential diagnosis in adult neuro clients.
  • Understanding of multi‑disciplinary team working.
  • Knowledge of outcome measures within adult neuro client group.
  • Knowledge and understanding of Clinical Governance.

Desirable

  • Special knowledge/interest in aphasia and/or motor speech disorders.
