Job Summary
Part‑time physiotherapist available 20.5 hours per week. Join the Tavistock Community Therapy Team at Abbeyrise, located in the centre of the historic town of Tavistock. The role is part of the Livewell South West Community Rehab teams and involves work across the Livewell area. Flexible part‑time, job share options are available.
Location & Facilities
The Abbeyrise office is alongside the river, offering free on‑site car parking, hybrid pool cars and extensive IT support. The team works closely with Devon Social Care, District Nursing and Healthy Ageing Team colleagues, providing a holistic service. Car sharing and bus service links from Plymouth are available.

Responsibilities
- Work within the community setting as part of the Livewell South West Community Rehab teams.
- Deliver all aspects of physiotherapeutic assessment for patients with diverse or complex physical, psychological, cognitive and behavioural conditions.
- Formulate and deliver an individualised physiotherapy treatment programme including manual therapy, therapeutic handling, patient education, exercise (individual/group) and alternative options.
- Manage clinical risk and exercise professional and legal accountability for all own work.
- Participate actively in the multidisciplinary team to provide comprehensive care.
- Lead or contribute to the implementation of practice changes and service protocol updates.
- Supervise, educate and assess Physiotherapy students, working with universities to meet degree‑level standards.
- Provide spontaneous and planned advice, teaching and instruction to patients, relatives, carers and other professionals.
- Advise on and prescribe appropriate rehabilitation aids and equipment; recommend low‑level adaptations.
- Initiate, organise and attend multidisciplinary/multi‑agency case conferences, liaison meetings and patient reviews.
- Assess capacity, obtain valid consent and document accordingly; comply with confidentiality, child protection and vulnerable adult procedures.
- Support and mentor junior staff; promote evidence‑based practice and continuous professional development.
Experience – Essential
- Member of an MDT with flexible working ability.
- Extensive clinical experience in health and/or social care.
- Physiotherapy treatment planning experience.
- Documented evidence of CPD.
- Relevant post‑registration experience.
- Multi‑pathology assessment.
- Home and community assessments.
- Effective caseload management and discharge planning.
- Management, leadership and supervisory skills.
Experience – Desirable
- Clinical supervision and appraisal of staff.
- Risk assessment in a variety of settings.

Qualifications – Essential
- Diploma / Degree in Physiotherapy.
- HCPC Registration.
- Ability to travel independently to Livewell Southwest sites and community locations, including areas not served by public transport.
