£30,000–£33,000 per annum. Full time, 40 hours per week. Hull branch.

Responsibilities:

  • Allocate care workers to customer visits using effective rostering, ensuring continuity of care and taking into account locations, skills required and customer preferences.
  • Take responsibility for new referrals from private customers and social services teams, ensuring full information is received and appropriate care planning is undertaken.
  • Assist the Branch Manager with identifying further business opportunities outside of the current client base.
  • Match care workers to customers, ensuring care workers are notified of all conditions relating to the assignment, including care worker’s rate of pay.

Qualifications:

  • 3–6 months experience as a care coordinator in a domiciliary care setting (essential).
  • Minimum 12 months domiciliary care experience within the last 3 years (essential).
  • Level 3 NVQ in health and social care, working towards Level 4; if not, must be signed up on appointment (essential).
  • PASS knowledge and CP knowledge (desirable).
  • ICT knowledge (desirable).
  • Understanding of the 5 Key Lines of Enquiry (desirable).
  • Driving licence and own vehicle (essential).
  • Working every other weekend (on‑call backup).
  • Good IT, communication and office skills (essential).
