As a Care Coordinator at LTS Social Care Services, you will play a central role in ensuring the effective delivery of high-quality community care. This position involves coordinating care plans, supervising care staff, supporting service managers, maintaining compliance with SSSC guidance, and ensuring service users receive person-centred, outcome-based support.
Key Responsibilities:
- Provide leadership and support to care staff within the service.
- Ensure care is delivered in line with best practices, company policies, and regulatory standards.
- Manage weekly rotas and allocate shifts based on service needs.
- Oversee staff performance and conduct regular supervisions and meetings.
- Participate in assessments and reviews of service user care plans.
- Ensure continuity of care, stepping in to provide direct support when required.
- Liaise with health and social care professionals to coordinate services effectively.
- Maintain accurate records, report incidents, and contribute to quality assurance.
- Actively support the recruitment process, including interviewing prospective care staff.
- Participate in on-call duties on a rotational basis.
- Promote the growth of the service by updating stakeholders and identifying new referrals.
- Work closely with the Registered Manager to improve operations and support business development.
Essential Requirements:
- Strong IT literacy and confidence using scheduling and record-keeping systems.
- Full-time availability (39 hours per week).
- SVQ Level 3 in Health & Social Care (or willingness to work towards the qualification).
- Full UK driving licence and access to a vehicle.
