LOROS Hospice is expanding its community services and seeks an experienced Advanced Clinical Practitioner to join the ICSPC Team, delivering expert palliative and end of life care across Leicester, Leicestershire and Rutland.
Working alongside palliative medicine consultants and neighbourhood teams, the post involves autonomous assessment, prescribing, care planning and leadership across hospice and community services to improve timely discharge and reduce avoidable hospital admissions.
