- Lead and support our Hospice at Home team to deliver highquality care in patients homes
- Manage referrals, coordinate care, and work closely with multidisciplinary partners
- Provide expert clinical leadership, assessment, and guidance to staff, patients, and families
- Help prevent unnecessary hospital admissions, enabling patients to be cared for at home
- Support service development, governance, staff training, and clinical standards
- Line manage HCA and RN teams, providing supervision, mentorship, and performance reviews
