Overview
HIM Auditor – Chart Integrity
The HIM Auditor is a subject matter expert in the integrity of the Electronic Health Record (EHR) with regards to clinical documentation and scheduling/registration and provides ongoing operational support to ensure the integrity of the EHR. This includes auditing for accurate and timely completion of patient documentation, scheduling and registration errors, and maintaining the integrity of the regional central patient index (CPI). Auditors are responsible for identifying and diagnosing errors, determining corrective actions, and either correcting or overseeing resolution of errors.

Responsibilities
Responsibilities include auditing for accurate and timely completion of patient documentation, identifying and diagnosing errors, determining corrective actions, and either correcting or overseeing resolution of errors to maintain chart integrity.
Qualifications
- Successful completion of high school diploma
- Successful completion of a Health Records Clerk Certificate, Health Care Office Assistant Certificate, Health Care Office Admin Certificate, Health Information Management Diploma, Medical Office Administration Diploma or an equivalent combination of education, training and experience as determined by the Hospital
- Certified Canadian Health Information Management (CHIM) Professional preferred
- Minimum 1 year of recent and relevant experience if CHIM Professional or minimum 3-5 years of relevant experience as determined by the hospital
- Proficient working knowledge of LHSC policies, privacy and legal requirements while adhering to patient confidentiality standards
- Knowledge of the Transcription System to a degree required to support both administration and troubleshooting tasks
- Knowledge of the EHR System and legislated documentation standards to a degree required to identify deficient or incorrectly posted documentation and teach users to enter it correctly, including understanding all the nuances associated with all the different methods of posting notes to the EHR as they all function differently
- Knowledge of scheduling and registration practices required to ensure patient safety and fulfill Ministry reporting obligations for MIS (financial activity) and CIHI coding (abstracted visits)
- Sound critical thinking and knowledge of HIM best practices and LHSC policies to independently conduct detailed investigations into both paper and electronic based health records
- Proficiency with computer-based tools and systems such as Microsoft Office Suite, Excel and Cerner
- Proficient problem-solving skills to proactively identify teams that will be impacted by changes to a health record and lead coordination between teams where required
- Attention to detail to alter electronic health records accurately to improve chart integrity, including merging records and moving records between visits and/or patients and to assess quality of clinical documentation and identify missing or inaccurate information
- Strong communication skills to communicate professionally with staff, leaders and professionals throughout the region
- Ability to record comprehensive documentation of auditing procedures, activities and findings
- Maintain detailed knowledge of medical terminology and abbreviations preferred
- Maintain knowledge and understanding of medico-legal aspects of chart completion as required by applicable acts (preferred)
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC\\\’s vision, purpose and values
