London Health Sciences Centre (LHSC) is a world-class academic health sciences centre located in the southwestern Ontario city of London. The Pharmacist is responsible for the safe and effective use of medications for patients admitted to London Health Sciences Centre. In meeting this responsibility, the Pharmacist reviews and monitors drug therapy; interacts with patients, physicians, nurses, and other allied health professionals to resolve drug related problems; and counsels and educates patients regarding drug therapy. Pharmacists also participate in the training and preceptorship of undergraduate and resident Pharmacist students.

Rate of Pay: $54.93 per hour - $68.67 per hour

Regular Full-Time Hours of Work: 37.5 hours per week

  • VH Thoracic Surgery, ENT, Plastic Surgery
  • VH Adult Intensive Care Unit
  • VH Pediatric Gastroenterology, Nephrology and Pain Clinic
  • VH Pediatric Diabetes/endocrinology, Immunology, Rheumatology
  • VH Neonatal Intensive Care Unit, Adult Gynecology and Obstetrics
  • UH Adult Infectious Disease and Antimicrobial Stewardship
  • UH Neurology
  • UH Team 1 Medicine

Temporary Full-Time Hours of Work: 37.5 hours per week

Duration: up to 6 - 18 months

Area of Assignment:

  • VH Adult Infectious Disease and Antimicrobial Stewardship
  • VH Adult Emergency Department
  • UH Multiorgan Transplant Program

Regular Part-Time Hours of Work: No guarantee of hours

Areas of Assignment:

  • VH Subacute Medicine Unit
  • VH Drug Distribution/ Order Verification

Qualifications:

  • Baccalaureate Degree or Doctorate Degree (PharmD) in Pharmacy
  • Registered as an A Pharmacist with the Ontario College of Pharmacists (OCP)
  • Professional Liability Insurance as prescribed by the Ontario College of Pharmacists
  • Up to one (1) year of recent, related experience
  • One (1) year of recent, related experience preferred
  • Hospital Pharmacy Residency certificate (Accredited Canadian Pharmacy Residency: ACPR) preferred
  • Successful completion of an OCP-approved injection training course preferred
  • Prescribing for Minor Ailments Certificate preferred
  • Current First Aid and CPR-Level C certification for which you can provide proof upon request
  • Proficiency in OCP Standards of Practice for Pharmacists
  • Proficiency in NAPRA Professional Competencies
  • Proficiency in Cerner, PharmNet, and Powerchart System
  • Proficiency in Kroll Pharmacy Computer System
  • Proficiency in Powerchart Oncology (Oncology Only)
  • Proficiency in the Microsoft Office suite of applications
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC's vision, purpose and values
  • Demonstrated ability to attend work on a regular basis
