Overview

London Care Support Service is seeking a passionate and experienced Care Manager to join our team. The ideal candidate will be responsible for overseeing and coordinating the delivery of care services to our clients, ensuring their physical, emotional, and social needs are met.

Responsibilities

  • Conduct comprehensive assessments of clients\’ needs and develop personalized care plans.
  • Collaborate with healthcare professionals to ensure the effective implementation of care plans.
  • Ensure all required CQC standards are adhered to and undertake staff supervisions and appraisals in accordance with CQC requirements.
  • Lead and supervise a team of care professionals, providing guidance and support.
  • Foster a positive and collaborative team culture, promoting continuous professional development.
  • Serve as the primary point of contact for clients and their families, addressing concerns and providing support.
  • Monitor and evaluate the quality of care services, identifying areas for improvement and implementing necessary changes.
  • Ensure compliance with relevant regulations and standards.

Requirements

  • Diploma Level 5 Diploma in Leadership for Health and Social Care.
  • Passion for delivering exceptional care.
  • Good working knowledge of CQC essential standards and the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
  • Experience working with clients who require additional support or complex needs.
  • Previous experience as a Home Care Manager.
  • Excellent organisational and planning skills.
  • Strong communication and interpersonal skills.
