About The Role and Ideal Candidate

As a key front‑line service delivery role within the Brokerage Service, you will arrange specialist care for children, young people and adult residents with care and support needs. You will source and secure high‑quality placements in supported living, care homes, residential or nursing homes, and other place‑based services, ensuring that each placement aligns with the outcomes identified in individual support plans.

About You

The post holder will work under pressure, prioritise tasks and meet tight deadlines in a fast‑paced environment.

Key Skills and Experience Required

  • Detailed knowledge of Adult Social Care services such as residential and nursing care and Children with disabilities, as well as domiciliary and reablement care.
  • Deliver service activities and manage caseloads under the supervision of more experienced staff to ensure intended outcomes for children, young people and adult residents and the Council are delivered within agreed service standards.
  • Sourcing Care Packages: Identify appropriate services including residential/nursing homes, domiciliary care, supported living, respite care and other place‑based services.
  • Negotiation & Value for Money: Negotiate fees with providers to ensure services fit within the council’s budgetary frameworks and provide the best value.
  • System Maintenance (e.g., Mosaic): Accurately record, monitor and update care packages and purchasing records on IT platforms.
