Senior Social Worker – Richmond, Response & Rehabilitation Team (RRRT)

Full Time, Permanent, 36‑hour week. Location: Civic Centre 44 York Street, Twickenham, TW1 3BZ.

We are looking for an experienced senior social worker to lead complex discharge planning, oversee hub management and champion strengths‑based practice within our high‑performing Hospital Discharge Team.

As a Senior Social Worker you will work within a collaborative MDT environment to ensure safe, timely, person‑centred discharges and maintain high‑quality professional practice.

  • You will need
  • Strong verbal and written communication skills and ability to represent Adult Social Care confidently in professional forums.
  • Experience supporting management to deliver a person‑centred service promoting independence and wellbeing for residents leaving hospital.
  • Duty, formal supervision, coordination of referrals, oversight and clear decision making and attending hospital discharge planning meetings.

Experience as safeguarding adults manager (SAM) or willingness to train.

  • Experience of hospital discharge (or willingness to gain experience).
  • Ability to complete Care Act and Mental Capacity Assessments.
  • Previous experience or keen to learn how to supervise others.

Essential Qualifications, Skills and Experience

  • SWE registration with a recognised social work qualification and evidence of ongoing professional development.
  • Supervision skills – ability to provide reflective, supportive and accountable supervision to staff.
  • Duty and workflow management – experience managing duty referrals and making confident, proportionate decisions in a fast‑paced hospital discharge setting.
  • Care Act and MCA competence – strong understanding of Care Act duties, Mental Capacity Assessments and best‑interest decision‑making.
  • Integrated MDT working – proven ability to work collaboratively with health partners, therapy teams and community services to deliver safe, timely discharge pathways.
  • High‑quality communication – excellent written and verbal communication with confident, evidence‑based decision‑making.
  • Strengths‑based practice – commitment to promoting independence, wellbeing and person‑centred outcomes.
