Objective of role
Would you like the opportunity to take on new training and development opportunities that will support your career progression? You will join our Adult Social Care Locality Teams, supporting residents aged 18+ with physical disabilities and older people with physical and mental health needs. You will play a key role in delivering strengths-based, personalised care that promotes independence, wellbeing and positive outcomes. Working within a collaborative, multi-disciplinary environment, you will apply your professional expertise to safeguard adults, manage risk and ensure high-quality, value-driven services.
About the role
- Undertake strengths-based assessments, reviews, and develop personalised care and support plans to promote independence.
- Work in partnership with residents, carers and professionals using a person-centred approach.
- Identify, assess and manage risk, including safeguarding interventions.
- Apply relevant legislation, guidance and policy in your practice, including safeguarding responsibilities.
- Work collaboratively across multi-disciplinary teams to achieve positive outcomes.
- Demonstrate a proactive, can-do attitude and the ability to make sound judgements, including in emergency situations.
You will be committed to delivering high standards of service, achieving value for money and putting residents at the heart of everything you do.
Essential Qualifications, Skills and Experience
- Qualified Social Worker with current Social Work England registration.
- Post-qualification experience in a statutory social care or health setting.
- Experience of assessments, care planning, safeguarding and risk management.
- Good knowledge of adult social care legislation and safeguarding responsibilities.
- Proven ability to work in partnership and achieve positive outcomes.
- Strong communication, decision-making and time management skills.
