The post holder will effectively triage residents eligible under the Care Act, connecting them to activities that promote meaningful relationships and a sense of purpose. The role provides intensive, time‑limited support as a preventative intervention, addressing the adverse effects of isolation and loneliness. This will include working directly with residents in the community over a six‑week period to identify their interests and support them to engage in suitable local activities. Through this approach, the post holder will help to sustain residents’ independence, enhance their overall quality of life, and contribute to reducing long‑term demand on Health and Social Care services.
