Transition Team
The Merton‑Transition Team provides support to young people and their families with additional needs as they prepare to move from childhood to adulthood. The team works closely with a range of partners across Children’s and Adult Services, SEND, community health, and mental health services.
Responsibilities
- Support individuals to live well and maintain independence.
- Collaborate with partners to enable people to live fulfilling lives in their homes.
- Connect clients socially within their communities and introduce adjustments such as handrails or home care services.
- Empower clients to make informed decisions about their care.
