You will work in a supportive team with knowledgeable managers and collaborate with partners across services.

Transition Team

The Merton‑Transition Team provides support to young people and their families with additional needs as they prepare to move from childhood to adulthood. The team works closely with a range of partners across Children’s and Adult Services, SEND, community health, and mental health services.

Responsibilities

  • Support individuals to live well and maintain independence.
  • Collaborate with partners to enable people to live fulfilling lives in their homes.
  • Connect clients socially within their communities and introduce adjustments such as handrails or home care services.
  • Empower clients to make informed decisions about their care.
