We are looking for a proactive, conscientious, and flexible Social Worker to join our Community Duty and Safeguarding Team. This is a fast‑paced service as part of the front‑door teams for Adult Social Care. You will play a key role in responding to referrals, managing risk, and supporting adults to remain safe and independent within the community. The team works across a broad range of areas including safeguarding, older people, physical and learning disabilities with complex social care needs.
You will need to be adaptable and open to supporting across different workstreams, depending on service demand. You will act as a key point of contact and liaison between internal teams and external partners, including health professionals, care providers, and other agencies. Strong communication and relationship‑building skills are essential, as you will be working collaboratively to deliver timely and effective interventions.
While experience in safeguarding or duty work is desirable, we welcome applicants with transferable skills such as risk assessment, case management, partnership working, and the ability to analyse and apply information to support decision‑making. You will be supported to develop your knowledge and skills within the role. You will be responsible for triaging referrals, managing urgent situations, and undertaking timely interventions to safeguard adults at risk. You will work with individuals aged 18 and over who may have a range of needs, including physical health conditions, learning disabilities, and other vulnerabilities. A key part of the role involves undertaking safeguarding concerns, completing reviews, and developing and updating support plans to ensure individuals receive appropriate, person‑centred care.
You will be expected to apply a strengths‑based approach while managing risk effectively. You will also be required to carry out urgent home visits to assess needs, respond to safeguarding concerns, and ensure immediate risks are addressed. The ability to make sound professional judgments in dynamic situations is essential.
You will also complete Mental Capacity Assessments in line with the relevant legal framework, ensuring that individuals are supported to make decisions wherever possible, and that best interest processes are followed where required. This role requires strong partnership working, as you will liaise with health professionals, care providers, housing, and other agencies to coordinate effective outcomes.
