About the Role:

We are seeking three motivated qualified social workers to join the Lambeth Hospital social work team, based at Kings college Hospital (KCH)

  • This team supports Lambeth residents who are admitted to KCH, GSTT and hospitals out of the borough.
  • The primary feature of work undertaken by staff in the Hospital team is to deliver strength based preventative support, where required assess and establish supports and safeguard patients.
  • The team undertake reviews of existing packages of care and consideration of placements
  • Hospital social workers carry out a variety of responsibilities, including referring people to rabblement where they are identified as potentially benefitting from maximising their everyday living skills, ensuring safe and timely decisions are made to facilitate discharges.
  • The team work closely alongside our MDT colleagues and voluntary sector organisations.
  • The Hospital is a fast paced team with a committed and enthused workforce who strive to achieve positive outcomes for our residents.
  • You will work closely with other teams in the borough and engage wider agencies to support the person.
  • You’ll need to adapt to a demanding and changing environment and have a commitment to partnership and collaborative working.

• The team carry out assessments or reviews with adults over 18 years with a range of physical health disabilities and vulnerabilities, so experience with these client groups is essential – as well as having a robust understanding of the Safeguarding procedures. The team support hybrid working, the requirement is to be on site 3 days a week with the option to work 2 days at home.
