About The Role

The First Contact Team, situated within Hounslow House, plays a pivotal role in the Adult Social Care department of the London Borough of Hounslow. As a Social Work Assistant, you will conduct Care Act assessments, perform home visits, manage communication with residents, and support their care needs. This team serves as the initial touch point for residents seeking support, emphasizing a strengths‑based approach to foster independence while providing necessary care and support. You will uphold human rights and act as an advocate for residents, integrating social work values into everyday practice. The department is committed to nurturing professional growth, offering robust learning and development opportunities and ensuring comprehensive support through supervision and training.

About The Team You’ll Be Working In

  • Carry out social care assessments and provide care management support by assisting residents and carers to identify their needs and desired outcomes, help them formulate a Care Plan/Support Plan, and, where indicated, use their Personal Budget to set up individually tailored services.
  • Protect the human rights of residents and carers, formulate protection plans, carry out safeguarding enquiries and participate in outcome meetings in line with the Council’s and Pan London Safeguarding Policy.
  • Work collaboratively with key stakeholders, including care providers, housing, financial services, health, volunteer sector and religious institutions; demonstrate due regard for equality and diversity among residents, partners and colleagues.
  • Report to the Deputy Team Manager and receive support from the Team Manager.
  • Operate within a hybrid model of working between Hounslow House, other suitable council premises and remote working; digital hardware and onsite ICT support will be provided.
