About The Role
The First Contact Team, situated within Hounslow House, plays a pivotal role in the Adult Social Care department of the London Borough of Hounslow. As a Social Work Assistant, one is entrusted with a spectrum of responsibilities, including conducting Care Act assessments, performing home visits, and managing communication with residents to address their care needs. This team serves as the initial touchpoint for residents seeking support, emphasizing a strengths-based approach to foster independence while providing necessary care and support. Central to the service\âs ethos is the upholding of Human Rights, ensuring that the team acts as an advocate for the residents, integrating social work values into everyday practice.

About The Team Youâll Be Working In
You will be instrumental in carrying out Social Care Assessments providing Care Management support by assisting residents and carers to identify their needs and desired outcomes, helping them to formulate a Care Plan/Support Plan and, where indicated, to use their Personal Budget to set up individually tailored services to achieve desired outcomes.
You will be key in protecting the Human Rights of our residents and carers as much as working with them to promote their safety in line with the Councilâs and Pan London Safeguarding Policy. You will be instrumental in formulating protection plans, carrying out safeguarding enquiries and participating in outcome meetings.
You will work collaboratively with key stakeholders, care providers, housing, financial services, health, volunteer sector and religious institutions to name a few. There is an expectation that you will have due regard for Equality Diversity of our residents, partners and colleagues.
As a social work assistant, you will be reporting to the Deputy Team Manager and will also be supported by the Team Manager.
Hounslow Council are operating a Hybrid model of working based between Hounslow House, any other suitable council premises, and remote working. Hounslow House is a modern building an digitally advanced. We have onsite ICT support. All the digital hardware shall be provided to you, which will allow you to work flexibly.
About You
- Completing Care Act assessments to identify whether residents are eligible for care and support from the Local Authority.
- Completing duty tasks such as making telephone calls to establish residentsâ needs, completing referrals for physio input and any other tasks required to promote the wellbeing of residents.
- Completing Safeguarding alerts and identifying whether Section 42 enquiries are required.
- Working effectively as part of a team and contributing to a collaborative environment.
