About The Role
This is a fantastic opportunity to work as part of a collaborative MDT team to support residents who are either approaching the end of their life or are preparing for how to manage their palliative care. This role is based at Meadow House in Ealing Hospital and you will form part of an MDT with colleagues from Health and Social Care to assess Hounslow residents who are open to the palliative care team. Most residents who you will assess will have recently received a terminal diagnosis and part of your role will be to support them, along with their family, with the challenges that this brings. You will undertake Care Act assessments to identify the level of support required from the Local Authority to meet their needs. This role has huge potential for the successful applicant to support residents through the most challenging times in their lives. It is a privilege to work with people that are at this stage and your role will also be preventative, in trying to ensure that, where possible, care needs can be met to delay the onset of acute care and support needs. You will have the ability, within this role, to develop and maintain relationships with patients, carers and families using the strengths-based approach in order to allow, carers and others involved to adjust to their changing situation.
To support patients and carers formulate plans for end-of-life care including participation in advanced decisions is about protecting and supporting residents’ human rights. As such, this role is well suited to Social Workers who place human rights at the centre of their work and tirelessly advocate for residents’ wishes to be promoted and respected throughout all stages of their lives.
About The Team You’ll Be Working In
- Completing Care Act assessments and working with people who require support with their palliative care planning.
- Completing Mental Capacity Assessments.
- Understanding a strengths-based approach to social work and demonstrating your commitment to human rights through all the work you do.
- Working effectively as part of an MDT team and contributing to a collaborative environment.
About You
You will have contributed to a positive team culture and be a strong team member, with an emphasis on achieving collective goals and supporting colleagues on a daily basis. You will bring experience of delivering positive outcomes for residents that you have worked with and demonstrate examples of where you have upheld human rights. You will have sound and detailed knowledge of legislation such as the Care Act 2014, Mental Capacity Act 2005 and Human Rights Act 1998 to ensure compliance with statutory duties that are in place for Adult Social Care services. You’ll have to demonstrate your ability to work to tight timescales in a fast-paced environment and be able to flexibly manage your day-to-day priorities.
Qualifications:
- Social Work qualification required and/or awaiting certificate from Social Work England.
- Enhanced DBS will also be required.
