About The Role
- To promote a philosophy of putting customer needs first at every opportunity and to ensure the arrangements put in place are actioned.
- Provide advice, support and assistance to the CRS Manager and other senior officers in LBH and HRCH (Hounslow and Richmond Community Health Trust) on relevant Council, NHS and CRS strategies, policies and projects.
- To support performance management and undertake service reviews and quality monitoring visits, highlighting when further support or equipment is needed to help vulnerable residents achieve their goals.
- To work flexibly, covering on call duties during unsociable hours, evenings, weekends and Bank Holidays in order to support vulnerable residents and their carers and staff via mobile phone contact as well as through direct face to face contact.
About The Team You’ll Be Working In
We are seeking to recruit an enthusiastic and self‑motivated person to undertake the role of an Assessor for the Community Recovery Service. The Hounslow Community Recovery Service marks an exciting step towards health and social care working together. The Community Recovery Service is an integrated service with Hounslow and Richmond Community Healthcare NHS Trust and aims to support people's independence following an injury, illness or a change in life circumstances and living at home. The team provides a period of rehabilitation and reablement to help local people in Hounslow recover and regain their independence. It brings together different health and social care professionals into a single integrated team.
This is an exciting opportunity to develop skills in partnership working, performance management, mentoring and providing supervision of Rehab Assistants. You will be given the opportunity to be creative in this role, developing strategies and practices improving ways of working. You will receive significant support from the CRS Co‑Ordinators and the Registered Manager. You will need to work flexibly, in order to support vulnerable residents, their carers and staff in order to meet the needs of the service, a seven‑day service 8.00 a.m. to 22.00 p.m.
About You
You will be passionate about providing person centred care and a passion to support the rehabilitation journey for residents living in the London borough of Hounslow.
Your Skills
- To carry out assessments and fully consult with clients regarding individual goals and the preparation of their support plan, including assessment for basic equipment and Telecare/Telehealth if required in line with reablement and promoting independence.
- Ability to use business systems, information technology, spreadsheets and write reports.
- Ability to work flexibly.
- Ability to work independently.
