We are seeking permanent and part‑time staff to work as CRS Rehab Assistants in the Hounslow Community Recovery Service (CRS). The CRS aims to support people in Hounslow to regain independence after injury, illness or other life changes, through rehabilitation and reablement delivered by an integrated team of health and social care professionals.
The role will provide flexible 7‑day cover from 08:00 to 22:00, supporting vulnerable residents in community settings.
Responsibilities- Carry out personal care support as required—washing, dressing, toileting, bathing, transfers, medication administration and other care tasks to help clients maximise independence.
- Meet clients in their homes or community settings, safely and appropriately, and carry out risk assessments and follow lone‑working protocols as needed.
- Demonstrate a basic understanding of the effects of disability and apply this knowledge during client interventions.
- Carry out and contribute to client assessments, providing feedback on occupational performance as delegated by the occupational therapist or physiotherapist.
You will work alongside Therapists, Social Workers, Nurses and other multidisciplinary professionals within CRS.
Essential Skills & Qualifications- Ability to teach practical skills to clients.
- Basic understanding of the effects of disability.
- Understanding of enabling clients through the therapy process.
- Ability to perform moderate to intense physical effort throughout the day and manage concurrent activities.
- Enhanced DBS clearance.
- NVQ Level 2, Care Certificate or equivalent experience.
Training will be provided.
