London Borough of Hounslow is seeking a Family Support Practitioner to work with children and families in need. You will assess and support families, contributing to integrated responses and collaborative working with colleagues and partner agencies.
You will work within our Family Help Team, providing targeted and statutory support to children and young people aged 0-18 (up to 25 with additional needs) and use CIN/TAF plans to deliver coordinated support that improves outcomes and prevents.
