About The Role
This is an exciting time to work in the Older Persons Placements and Brokerage Team in Hounslow. A new opportunity has arisen for an innovative individual who is passionate about ensuring the best value and care is provided for vulnerable adults in the borough.
As a placement team, our aim is to ensure that high quality services are delivered through effective brokerage outcomes for our residents, facilitating timely discharges from Hospital and working with colleagues in NWL ICB to source care for residents on continuing care pathway.
You will also be key in conducting provider audit visits, contract management meetings and ad-hoc visits to providers to assess standards.
You will have the opportunity to develop your skills in service development, review and evaluation.
About The Team You’ll Be Working In
We contract manage those providers that are based within the borough, using audits, unannounced visits, contract monitoring meetings, and quality alert investigation. We have developed a positive working relationship with our providers and continue to develop the market.

  • You will bring experience of delivering solutions to potentially complex problems.
  • You will need to manage your time very carefully, balancing a large number of competing, and seemingly impossible requests for your time and attention.
  • You will have sound and detailed knowledge of the local adult care marketplace.
  • You know how to communicate, expertly, with both internal colleagues (Care Professionals) and external stakeholders such as care homes, home care agencies and the NWLCCG clinical leads.
  • You will have contributed to the process of discharging medically optimised patients from local acute settings at rates which are commensurate with maintaining the ASC corporate budget.
