About The Role
This is an exciting opportunity to be part of the Leaving Care Team as an After Care Personal Advisor, providing practical and emotional support to young people who have left, or preparing to leave, local authority care. The role focuses on helping care leavers transition successfully into independent adulthood by promoting their wellbeing, education, employment, and life skills.

  • Responsible for developing close and effective working relationships with key agencies to facilitate and co-ordinate multi-agency responses to identified needs of care leavers.
  • Act as a consistent and reliable point of contact for young people leaving care, offering advice, advocacy, and emotional support.
  • Work with each young person to create, and implement their Pathway Plan, ensuring it reflects their goals, needs, and aspirations.
  • Support young people in accessing education, training, and employment opportunities.
  • Assist care leavers in securing and maintaining appropriate accommodation, ensuring they understand tenancy responsibilities and budgeting.
  • Provide guidance on health and wellbeing encouraging access to health services and promoting positive lifestyles.
  • Help young people develop independent living skills, such as budgeting, cooking and managing appointments.

The Team You’ll Be Working In
The team provide advice, guidance and practical and emotional support to young people 18 -25-year-olds, which includes former Unaccompanied Asylum-Seeking Children (UASC) who are preparing to leave or have left local authority care. The key aim of the team is understanding the challenges faced by care leavers and principles of corporate parenting. Knowledge of relevant legislation (e.g. Children Act 1989, Children Leaving Care Act 2000). Advocate on behalf of young people where necessary to ensure their voices are heard and their rights are upheld. Maintain accurate and up-to-up-date records of contact, progress, and outcomes in-line with safeguarding and data protection policies. Participate in team meetings, supervision, training, and service development activities. Capability of understanding and acting upon legislation concerned with housing and homelessness. Prepare, update, review and implement pathway plans in conjunction with the supervising manager and ensure parallel planning where applicable.

About You
Minimum 2 years working with young people in a supporting role in either a voluntary or statutory setting. Believing in care leavers and building on existing skills and aspirations can motivate them in their own abilities to succeed which can go along way in helping them overcome disadvantages in life. Adequately trained in monetary matters in order to better support Care Leavers in their choice of financial products and services. Ability to recognise and respond appropriately to issues of risk and safeguarding. Knowledge of the Health, Education, and Training.
