London Borough of Hillingdon is seeking an Annual Review EHC Co-ordinator to guide children, young people and families through the Education, Health and Care assessment process, coordinating annual reviews to keep EHCPs focused on positive outcomes.
You will manage a caseload of 0–25 year-olds with SEND, chair meetings, draft plans, and work with education, health, social care and voluntary partners to support transitions to adulthood. Hybrid role with two days in the office weekly.
