About the Integrated Discharge Service
The Integrated Discharge Service provides a service to Hackney residents. Residents are assessed, whilst in hospital across all London Hospitals and beyond, for the most appropriate and safe discharge destination and care and support plan. We focus on safe discharges that respect the legal rights and expressed wishes of all individuals. The team works in partnership with the multi-disciplinary teams within hospitals and attends multi-disciplinary meetings on their aligned wards. The service is inclusive to those in acute settings who are assessed as having a learning disability, mental health diagnosis, or dementia. It also supports residents with no recourse to public funds who have eligible care and support needs. The service operates seven days a week, and staff are invited to contribute to weekend working if they so wish.
Community Reviews are completed collaboratively with the resident, family members, carers, and other involved professionals, to ensure that their long-term care and support needs are assessed and that any newly arising concerns or needs are addressed. The team is separated into a Homefirst and Reablement team and a Complex casework team, with a Team Manager leading each pathway.
- Homefirst and Reablement Team Manager (IDS): Leading a Team of social workers, Occupational therapists, OT assistants and Review officers to support residents in hospital to receive a timely discharge via either the reablement pathway, a restart of their Package of care or a new care and support plan. Working closely with partner agencies including the NHS, Age UK, Housing and Extra Care.
- Complex Casework Team Manager (IDS): Leading a Team of social workers who are working with residents in hospitals with more complex needs who may require safeguarding, an interim placement or a short term Reablement assessment flat in order to safely meet their needs. The Team Manager will work closely with their counterpart in IDS to ensure a consistent approach in practice and operational processes. In addition, the team manager will work closely with partner agencies reinforcing an MDT approach and have a sound knowledge of the legal framework relating to Care Act Assessments, Human Right Act, Mental Capacity Act.
Candidates Will Need To Demonstrate
The post of Team Manager is fundamental to leading and developing the practice of the teams, ensuring that we are providing a safe and efficient service to all of our residents. Experience and enthusiasm to lead the team through transformational improvements to our service area. A commitment to integrated working across health and social care is essential. An ability to create and maintain positive relationships with partner agencies. A commitment to recruitment and retention of staff and growing our permanent workforce. Experience of managing social care staff and applying HR policies and procedures. Experience of analysing and reporting on performance through the use of data and dashboards.
Qualifications And Professional Registration
You will need to be a qualified Social Worker and be registered with Social Work England. You will also have a significant level of post qualification experience within statutory Adult Services as a senior practitioner or team manager level. You will need to have experience of providing supervisory / management support to Senior practitioners and experience of working with a range of adults that are eligible for adult social care. You will be required to work closely with hospital discharge leads, ICB representatives, partner agencies to deliver integrated discharge pathways and promote a culture of integrated working and high-quality person centred practice.
