We are looking for Discharge Social Workers and Community Social Workers. Hackney is one of the most dynamic and progressive local authorities in the UK, located in the heart of London. The Integrated Mental Health Service (IMHS) works in close collaboration with East London NHS Foundation Trust under Section 75 partnership arrangements to provide coordinated, person‑centred support in line with the Care Act 2014.
About the Role
We are recruiting for two Discharge Social Workers and three Community Social Workers to join our newly developed teams. These roles offer an opportunity to work within an integrated and multidisciplinary environment focused on recovery, safeguarding, reablement, hospital discharge and promoting independence for adults with mental health needs across Hackney.
Key Responsibilities
- Provide older people with mental illness and their carers with a clear and accurate assessment in line with personalisation principles of the Care Act 2014; planning, delivering and reviewing services and outcomes with individuals, their networks and support providers through multidisciplinary team work.
- Carry out enquiries under Section 42 of the Care Act 2014 where adults may be at risk of abuse or neglect, ensuring high standards of safeguarding investigations and working with other agencies.
- Support service users and carers, identifying and utilising strengths, and integrate work with other agencies and voluntary sectors to maintain independence.
- Maintain a good working knowledge of the Care Act, Mental Health Act, Mental Capacity Act/DOLS and other social care legislation.
Qualifications and Experience
- Social Work England registered and holding a CQSW/DIPSW and an AMHP qualification or equivalent.
- Experience within a mental health setting and in multi‑disciplinary teamwork.
- Experience conducting full psycho‑social assessments and writing reports.
- Evidence of continual professional development.
- Strong understanding of prevention, reablement and recovery‑focused approaches; experience supervising, mentoring or supporting staff.
Other Requirements
- Enhanced DBS is required.
- Experience of working with adults with mental health needs in a health or social care setting.
- Relevant vocational qualification in health or social care is an advantage.
- Indicate if you are AMHP qualified or working towards becoming an AMHP.
