The role operates within the front door, offering focused short-term intervention to mitigate immediate risk and ensure that residents ‘wait well’ when further complex care management has been identified. Working alongside Occupational Therapists within a multidisciplinary team, you will respond daily to referrals arriving via the hub which deal with a broad range of Adult Social Care queries and referrals. The focus is on supporting the Integrated Care Network (ICN) and working collaboratively with health partners to offer short-term intervention in the community.

The Adult Social Care sector is undergoing transformation. The senior social worker will help facilitate these changes and lead the team forward. The role requires a suitably qualified, professionally registered person with experience of supervising and managing staff, performance management, and supporting staff through complex and challenging casework.

You must be able to maintain healthy relationships with partner agencies including health, voluntary sector, and commissioned services. Strong IT skills and competency in social care systems recording are required. Excellent communication, analytical and written skills, problem‑solving ability, effective prioritisation, meeting performance targets and systems management are essential.

As part of a supportive management team you will work to the requirements of the Care Act and Mental Capacity Act, meet best practice, provide regular supervision, training, career development and follow the Council’s appraisal model.
