London Borough of Bromley is seeking three experienced Social Care Practitioners to undertake planned reviews for adults receiving support within the borough. The roles are based at Bromley Civic Centre with hybrid working (minimum 2 days in the office).
You will carry out Care Act needs assessments and annual reviews, assess and coordinate support plans, and work with providers, housing and health professionals to ensure continuity of care for adults across various placements.
