Age UK Bromley & Greenwich is seeking a proactive Health & Wellbeing Facilitator to support residents living with long-term conditions to build knowledge, skills and motivation to manage their health independently. You’ll deliver self-management workshops, run condition-specific groups, and work with volunteers to ensure right support at the right time.
