The London Borough of Bexley is seeking a Reablement Officer to visit service users at home, assess needs and design personalised reablement plans that promote independence, confidence and skills. You will identify barriers and opportunities, and work to enable people to regain control of daily activities.

You will join a multi-disciplinary team, liaising with care agency staff, occupational therapists and social workers to support older people, including those with mental health needs.
