London Borough of Bexley is recruiting a Social Worker to join the Bexley Care Integrated Triage Hub. This role supports the delivery of a multidisciplinary screening and ‘see‑and‑sort’ service for vulnerable adults in the community, including older people and those with physical, learning or mental health needs.
Responsibilities
- Deliver a multi‑professional screening service and provide a duty response for the Complex Care Team.
- Work with Health colleagues and the Rapid Response Team to manage crises and emergencies.
- Assess the service user’s and their carer’s needs and devise practical, person‑centred programmes to improve quality of life.
- Conduct Safeguarding Vulnerable Adults investigations and work within the national agenda for health and social care integration.
Qualifications and Experience
- Registered with Social Work England, with post‑qualifying experience or a New‑Qualified Social Worker meeting the essential criteria.
- Comprehensive knowledge of the Care Act and related safeguarding legislation.
- Demonstrated commitment to person‑centred practice and the Social Work Professional Capabilities Framework.
- Experience in safeguarding investigations; additional qualifications as a Best Interests Assessor or Practice Educator are an advantage.
