Several permanent fullâtime and partâtime opportunities are available within the Bexley Complex Care Team.
Location: Bexley Civic Offices.
Responsibilities:
- Provide a multiâprofessional service for referrals received by Bexley ASC using a personâcentred, strengthâbased model of intervention.
- Work closely with health colleagues and other services across Adult Social Care and Health.
- Conduct ongoing assessment and case management of vulnerable adults in their homes, supported living and residential care.
- Minimise risks and support individuals to live more independently.
- Assess the needs of residents and their carers, and establish creative, yet practical care plans that promote choice, control and independence.
Qualifications:
- Experience as a Social Care Assistant who can work effectively within a multiâdisciplinary team and autonomously when required.
- Ability to work at a fast pace, with resilience to cope with the demands of a busy team.
- Positive, forwardâthinking attitude and ability to collaborate as part of a team.
- Background in the assessment and provision of social care highly valued.
- Appointment subject to Enhanced Disclosure/DBS outcome.
