Role Overview
This role involves conducting face-to-face assessments, including Mental Capacity Assessments, to support safe and timely discharges from the hospital. You will provide information, advice, signposting, and arrange home care and residential support where needed. Collaborating closely with NHS colleagues and health professionals, you contribute to effective discharge planning. Once individuals return home, their cases transition to our Adult Early Intervention Team for further follow‑up. You\'ll uphold a focus on well‑being, resilience, and independence, supporting adults and carers in achieving important outcomes.
