About this job

Do you like making a difference in people\’s lives? if so this is the role for you. We have 2 full time vacancies for the role of Community Navigator based in one of our community hubs. The role includes working with residents face to face in a range of community settings tohelp them to identify issues that affect their health and well-being and co-produce a simple personalised care and support plan to move residents from need to self-reliance. You will support residents by connecting them to community based activities, groups and services that meet their practical, social and emotional needs, including specialist advice services, and arts and culture, physical activity, and nature and green based activities. You will use coaching and motivational interviewing techniques to support people to take control of their own health and well-being. You will support the development of accessible and sustainable community offers by working in partnership with VCSFE organisations, and others to identify gaps in provision and take a community development approach to enable growth in community activities and groups. You will work with individuals or families who have limited self-sufficiency / resilience in areas from example low skills, unemployment, poor health, poor housing conditions, potential homelessness, family breakdown, domestic violence, substance dependency or are experiencing neglect or have learning disabilities. There will also be the opportunity to work on proactive assistance projects to target individuals or families with a range of identified need.

About you

As a community navigator you will need to have experience of delivering 1 to 1 support to residents, who sometimes have complex needs. You will need to have a commitment to completing training when you start the role. Having good listening skills are essential as you’ll build relationships with the people you are helping by listening carefully to what’s important to them and what motivates them. You’ll work with them to create personalised support plans that gives them control over their health and well being. You will work with a range of people from different cultural and social backgrounds and the ability to work confidently and effectively in a diverse and challenging environment is essential. You will also need to have excellent interpersonal skills to develop relationships with VCSFE organisations and the wider multi-disciplinary virtual team. You will need to be organised and be able to manage your own appointments. Having experience of using case management systems to record information would be very beneficial.
