A Support Worker is responsible for providing care in accordance with our purpose and our policies thus ensuring compliance with all relevant regulations and laws that apply to the business
To provide care and support in a professional, discreet and caring manner always ensuring that the Service Users rights to independence, privacy and dignity is maintained
To work without direct supervision in the home of the Service User providing high quality care and support including personal, social and domestic support whilst adhering to the Service Users individual Care Plan
Key Responsibilities
- Care Delivery
- Support Service Users with daily living activities as outline in their Care Plans always promoting independence, privacy and dignity
- Administer medication subject to your authorisation, ensuring accurate records are maintained
- Travel to Service Users’ homes within the Isle of Man in accordance with agreed rota
- Support Service Users in their community to include external activities and events
- Liaise with healthcare professionals, families, and other stakeholders to ensure coordinated care
- Perform tasks safely that involve physical activity, including lifting and supporting Service Users
- Be vigilant for signs of distress or anxiety in Service Users or any deterioration in physical and/or mental health, or safety of the environment, and to report these as a matter of urgency
- To summon appropriate assistance in an emergency
- To regularly read support plans and risk assessments, acknowledging changes to required care
- Maintain cleanliness of Service Users home
- Prepare food
- Undertake essential shopping, payment of bills
- Actively encourage Service Users, according to their capabilities, to participate in all aspects of daily living activities
- Develop and maintain personal contact with the Service User through talking and listening
- Provide support as part of a caring team, liaising with informal carers, other agencies or professionals as necessary
- Safeguarding and Safety
- Ensure that all clients understand the care and treatment choices that are available to them
- Assist Service Users with mobility problems and other physical disabilities, including incontinence, using aids and personal equipment i. e. hoists, assistive technology etc. in line with their individual care plans
- Report any concerns relating to significant changes in the Service Users health and support needs
- Contribute to the Service Users review process, along all involved parties, by using your knowledge of assisting the Service User
- Understand the arrangements for ensuring that Service Users are safeguarded against the risk of abuse
- Report any incidents, accidents or near misses
- Identify and report risk of infection or contamination risks or issues for further assessment
- Assist with the general standards of hygiene and cleanliness
- Ensure that equipment is provided as part of the Care Plan is safe and fit for purpose
- Adhere to actions identified in Risk Assessments
- Report any observed faults, defects or damage to the premises, fixtures and fittings
- Challenging poor and bad practice
- Compliance and Quality Assurance
- Ensure care delivery complies with the Minimum Standards and the Regulation of Care (Registration) Regulations 2013
- Comply with company policies, procedures, and regulatory standards
- Participate in internal/external audits, inspections and reviews
- Record Keeping and Documentation
- Maintain accurate and up-to-date records of care provided, incidents and observations
- Ensure confidentiality and security of Service User information in accordance with GDPR
- Prepare and submit reports as required
- Not disclose or discuss confidential information outside of your work, or within your place of work EXCEPT in the proper discharge of your duties
- Communication and Collaboration
- Communicate effectively with Service Users, families, and multidisciplinary teams
- Address and resolve Service User complaints and concerns promptly and professionally or escalate to your manager
- Promote positive relationships with Service Users and their families, enhancing Service User satisfaction
- Knowledge and Experience
- Participate in meetings as appropriate, attend regular supervision sessions and complete any required training
- Maintain an active Personal Development Plan
- Undertake the appropriate level of training and regular updates
- Seek out best practice and look at innovative ways to improve the quality and efficiency of service delivery
