Role: Healthcare Assistant - Community Nursing Network (Band 3) Band 3: £25,760 - £27,476 37.5 hours per week (part-time considered)
Responsibilities
- Support nurses with assessments and care planning.
- Deliver nursing-based care to prevent unnecessary hospital admissions.
- Promote self-care for patients with long-term conditions.
- Contribute to health promotion and innovative service improvement projects.
Work schedule: the service operates between 8:00 am and 9:00 pm, with core hours from 8:00 am to 5:00 pm.
Expected to work one weekend in four and two late shifts (1:00 pm – 9:00 pm) over a four‑week period.
Qualifications & Requirements
- Enthusiastic, motivated, and reliable.
- Experienced in wound care.
- Car driver essential and able to travel independently across the borough.
- Must be able to demonstrate right to work in the UK.
