Locala Health & Wellbeing invites applications for a Diabetes Support Nurse in the diabetes service. The post is a Band 5-6 development role, 32 hours per week, on a permanent basis, within the community.
You will work with GPs, primary care partners and other services to provide patient-centred diabetes care, promoting self-management and partnership working across primary care.
