In this highly autonomous role, you will take the lead in assessing, managing and supporting individuals with complex needs, using a case‑management approach to keep conditions stable, prevent unnecessary hospital admissions, and improve overall quality of life. You will play a vital part in shaping and sustaining a service that truly makes a difference. As a senior clinician, you will also champion our organisation’s values, ensure safe and high‑quality care aligned with CQC standards, and contribute to ongoing service development.
