Job Role Band 3: £25,760 - £27,476
28 hours per week – 1 in 4 weekends. Service operating hours 0800‑2000. HCA’s 0800‑1600 or 1000‑1800. Fixed Term/Secondment – To 31st March 2027.
Responsibilities
The Hospital at Home service commenced in October 2022 (formerly Frailty virtual Ward). It supports patients with acute needs who can have their care provided in the community rather than in a hospital setting. The service operates 7 days a week between 8:00 and 20:00, delivering acute level care to Kirklees residents. The multidisciplinary team includes Frailty Nurses, a Physiotherapist and Health Care Assistants. You will support clinical tasks such as phlebotomy and clinical observations, and manage the remote monitoring service. We are seeking a Health Care Assistant to cover a maternity leave.
Essential requirements include:
- A full valid UK driving licence
- Access to a vehicle and business insurance
- An interest in acute support for frailty patients
A DBS check may be required.
